Therese Chan Tack, DO

Assistant Professor

Dr. Therese Chan Tack is an Assistant Professor of Clinical Medicine and Director of Telehealth in the Division of General Internal Medicine. She successfully led UC Primary care video visit expansion for two years and developed iterative workflows in collaboration with UCSF's Telehealth Resource Center.

She led UC Primary care quality improvement in colorectal cancer screening and hypertension management. This resulted in regional and national level poster awards.

She completed residency training at Loma Linda University Medical Center and served as Chief Resident. She earned a Master of Public Health degree at UC Berkeley with a focus in Health Policy and Management.

Before joining UCSF, she practiced both primary care and hospital medicine with Santa Clara Valley Medical Center for three years. She was recognized with their residency preceptor award.
