Therese Chan Tack, DO

Assistant Professor

Dr. Therese Chan Tack is an internal medicine physician at UCSF Primary Care at China Basin. She serves as Physician Lead for Video visits and has served as Quality Improvement Lead for a colorectal cancer screening and hypertension management.

Dr. Chan Tack received her medical degree at the Chicago College of Osteopathic Medicine. She completed residency training at Loma Linda University Medical Center and served as Chief Resident. She completed a Master of Public Health degree at UC Berkeley with a focus in Health Policy and Management.

Before joining UCSF, she practiced both primary care and hospital medicine with Santa Clara Valley Medical Center for three years. She was recognized with their residency preceptor award.

Dr. Chan Tack is Assistant Professor of Clinical Medicine in the Division of General Internal Medicine.
