Therese Chan Tack, DO

Assistant Professor

Dr. Therese Chan Tack is an Assistant Professor of Clinical Medicine and Physician Lead of Telehealth Programs for UCSF Health. She is closely involved in COVID-19 telehealth expansion across UCSF and its affiliates. She previously led DGIM's telehealth expansion efforts in 2019, having launched and developed video visits at UC Primary Care since 2017.

She led quality improvement (QI) projects in outpatient colorectal cancer screening and hypertension management. The effort led to regional and national level poster awards at SGIM and ACP.

She is an internal medicine specialist who provides primary care to adult patients and teaches residents. She joined the Division of General Internal Medicine (DGIM) at UCSF in 2016. Before joining UCSF, she practiced primary care, urgent care and hospital medicine with Santa Clara Valley Medical Center. She was recognized with their residency preceptor award.

She completed residency training at Loma Linda University Medical Center and served as Chief Resident. She earned a Master of Public Health degree at UC Berkeley with a focus in Health Policy and Management.
